Title: IPOST: Putting Impactful Conversations Into Action

Objectives:

Understand the POLST paradigm movement and Iowa’s IPOST
Understand the power of the IPOST conversation
Learn how to use the AsK, Teach, Ask model for having difficult conversations
Identify common pitfalls in difficult conversations
Identify common pitfalls with the IPOST form
Learn strategic language for navigating critical conversations

The IPOST form is a dynamic and actionable provider order form that has been in use in Iowa for over a decade. It is part of the national POLST movement which has been pushing to drive critical conversations, through shared decision making, and capture them in a provider order that is both transportable and clearly actionable towards the patients spoken goals and wishes. Understanding how to utilize the form is paramount to helping defragment a health system that is often riddled with challenges in communication between providers.

Dr. Tom Mouser has been a tertiary Hospice and Palliative Care physician through his role as the Chief Medical Officer at EveryStep for the past 12 years. He is dually boarded in Internal Medicine and Hospice and Palliative Care after completing residency at University of Iowa-Des Moines and fellowship training at Stanford University. Dr. Mouser has spent countless hours at the bedside of those with serious illness and chronic disease helping navigate complex medical decisions guided by the patient’s goals of care. He has served on the Iowa state IPOST task force helping promote understanding and better utilization since its inception.
