Position Overview
The Physical Therapist evaluates, plans, and administers evidence-based treatment services to restore, maintain, and improve patient function. This role collaborates with patients, families, providers, and the healthcare team to develop individualized plans of care and support successful patient outcomes.
Essential Responsibilities
Patient Evaluation & Treatment
- Perform comprehensive rehabilitation evaluations and develop individualized plans of care
- Provide evidence-based treatment interventions including gait training, strengthening, mobility training, and therapeutic exercise
- Utilize manual and non-manual therapy techniques as appropriate
- Monitor patient progress and modify treatment plans based on patient condition and outcomes
- Collaborate with patients, families, and healthcare team members throughout the rehabilitation process
Care Planning & Patient Education
- Establish realistic treatment goals and discharge planning strategies
- Educate patients and support systems on exercises, mobility needs, equipment, and continued care recommendations
- Utilize outcome measurements to assess treatment effectiveness and patient progress
Documentation & Compliance
- Maintain accurate and timely patient documentation
- Complete billing and reporting requirements accurately and efficiently
- Ensure treatment plans comply with regulatory and professional standards
Communication & Collaboration
- Communicate effectively with providers and interdisciplinary healthcare team members
- Promote positive collaboration across departments
- Report changes in patient condition appropriately and promptly
Patient Safety
- Follow patient safety and infection control protocols
- Utilize safe transfer and lifting techniques
- Promote safe treatment practices and minimize fall risk for patients and staff
Additional Responsibilities
- Perform other duties as assigned
